If you’ve been off work up to 7 days you can use the online SSP form for your employer SSP Form
You only need a doctor’s note if you’re unable to work and are ill for longer than 7 days.
We DO NOT provide fit-for-work notes
**Some employers may have their own policy that requires employees to obtain medical evidence that they are fit for work. Your employer should help you to arrange this privately. We cannot issue a fit note for this purpose**
