These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates & private prescriptions.
Appointments may be made for special examinations such as pre-employment checks, adoption, foster care and some others which reception can advise on.
A special appointment will be required and a fee will be charged for this service.
